About this role
OpenTrain is recruiting for a Hospitalist Clinical Documentation Reviewer to evaluate inpatient clinical records for accuracy, completeness, clinical appropriateness, and fidelity to the patient encounter. You will apply clinical judgment to H&Ps, progress notes, and discharge summaries, supporting a quality-focused review program through consistent adjudication and clear reasoning. This contractor role is designed for experienced hospitalists who can assess nuanced documentation issues independently and communicate defensible decisions in writing and verbally.
What you will do
Review inpatient H&Ps, progress notes, and discharge summaries against clinical accuracy and completeness standards. Adjudicate ambiguous or conflicting information and render clear, well-reasoned judgments. Identify clinically significant errors, omissions, and inconsistencies, then categorize them by severity. Apply structured rubrics and quality criteria consistently across cases. Provide concise written rationale for each adjudication decision. Contribute practical feedback that helps refine review guidelines and quality standards.
Required skills
An MD or DO degree with board certification in Internal Medicine or Family Medicine is required. Candidates should have at least five years of experience as an attending hospitalist or demonstrated leadership in documentation audit programs. The role requires deep familiarity with inpatient documentation standards and hospital quality assurance processes, the ability to evaluate clinical documentation and explain nuanced clinical reasoning clearly, and an understanding of how documentation affects coding, billing, and compliance. Candidates should be able to work independently in a remote contractor environment.
Helpful background
Experience with clinical documentation improvement programs is valuable, particularly when combined with documentation QA, audit, or broader clinical quality leadership. The work is compensated at an advertised rate of $100‑$150 per hour.
