The role — overview
OpenTrain is hiring a Pharmacy Prior Authorization Specialist to review AI-assisted pharmacy benefit management work for specialty medications, biologics, and other high-cost pharmaceuticals. You will evaluate prior authorization requests and AI-generated recommendations for accuracy, completeness, and payer compliance across commercial, Medicare Part D, and Medicaid coverage contexts.
This is a part-time contractor role requiring 20+ hours per week, for US-based contributors only. Compensation is $75 per hour, paid to contractors through OpenTrain.

What you'll do
Your core responsibility is to review prior authorization requests and AI outputs, reconcile them with clinical documentation and payer rules, and provide structured feedback that improves AI recommendations and downstream authorization outcomes.
Review clinical documentation and pharmacy benefit criteria for specialty drug authorizations.
Evaluate AI-generated prior authorization recommendations for accuracy, completeness, and payer compliance.
Work through step therapy and formulary rules when assessing recommendations.
Coordinate with prescribers, specialty pharmacies, and payers to secure timely medication authorizations.
Manage denial appeals, peer-to-peer requests, and exception workflows.
Track authorization status, approval rates, and denial trends to inform process improvement.
Annotate AI outputs and provide structured feedback for training datasets (evaluation ratings and text feedback).
Required qualifications
You must bring hands-on prior authorization and specialty access experience and be comfortable reviewing clinical records and payer policies to determine medical necessity and compliance.
Strong written and verbal English communication and high attention to detail are essential because your annotations will guide model behavior in sensitive clinical and payer decisions.
5+ years of experience in pharmacy prior authorization, specialty medication access, or pharmacy benefit management.
Deep knowledge of specialty drug formularies, step therapy protocols, and payer prior authorization criteria.
Familiarity with Medicare Part D coverage determination and exception processes.
Experience using specialty pharmacy platforms and PBM systems.
Strong written and verbal English communication and careful documentation skills.
Comfort evaluating AI-generated pharmacy recommendations and providing structured feedback.
Helpful background (not required)
Candidates with certifications or specialty pharmacy experience will be at an advantage, though these are optional. Familiarity with AI tools and previous annotation experience is helpful when reviewing model outputs.
Certified Pharmacy Technician (CPhT) or Certified Prior Authorization Professional (CPAP).
Experience with hub services, manufacturer copay assistance, and patient assistance programs.
Background in specialty pharmacy clinical areas such as oncology, rheumatology, or infusion services.
Prior experience evaluating or annotating AI-generated clinical or pharmacy content.
